Do I need a breast lift with augmentation?

Over time, you may develop stretched chest skin, with your breasts sitting lower on the chest wall, and are considering a breast lift. A breast lift does not increase breast volume, but lifts the breasts on the chest, and reshapes the breast tissue. It is common for us to remove some of the breast tissue in the lower pole that would weigh down the breast in the future. This often allows us to place a conservative size implant that is not too heavy. The nipple structure can be moved to be on the center of the restored breast mound. Adding the implant will help provide fullness in the upper pole and cleavage area. Combining the two often allows women to wear certain tops and dresses and not need a bra.

Internal Bra

In some cases, an internal absorbable surgical mesh is placed to support the breasts in their new location. The mesh is not permanent and does not harm the breast tissue. For more volume, implants can enhance breast size to provide fullness to the upper breast. The addition of the internal bra in particular helps support the added weight of the implant so that the breast does not start stretching back to its previous position. The mesh provides great short-term results, but most importantly gives longevity to the new breast shape. If implants are not placed with a lift, they can be added later if more volume and fullness is desired.

How does a breast lift with augmentation work?

During a breast lift with augmentation, the pocket is made for the implant, followed by the lifting surgery. The size of the implant can’t be too large, to avoid excessive weight pulling the repositioned breast which could cause asymmetry and malposition. A new opening is created to reposition the nipple areolar complex. The nipple remains connected to the breast tissue to preserve its blood supply and sensation. For excessively large areolas, the diameter is made smaller so that it will be proportionate with the breast.

Breast lift incisions

Breast lifts are performed with varying incision patterns. Some common incision patterns include:

Donut

Incisions around the areola (called circumareolar, or Benelli lift) is rarely used as it can result in a flattening of the breast and a widening of the areolas.

Lollipop

A “circumvertical, “short scar,” or “lollipop” pattern results in a closure around the areola and a  vertical closure down the breast to the natural fold below the breast. These scars heal very well and preserve round, youthful breast shape. In some cases, a small horizontal incision is made in the crease of the breast.

Anchor

Full circumareolar, vertical, and horizontal incision (referred to as anchor, inverted “T”, or Wise pattern) is used where there is more skin laxity or a very low nipple position.

Breast augmentation

Breast augmentation with a lift can be performed as a single procedure. Rarely, two separate procedures could be needed. The surgical “pocket” for the implant is created, followed by the lift, which will be based on the current breast condition. If you have chosen to have breast augmentation with lift, the implant will be placed during the lift procedure, typically placed beneath the pectoralis muscle. This brings additional support to the breast and works like an internal bra, for a more stable, long-lasting enhancement.

What is the breast augmentation with lift surgery?

Breast augmentation with a lift is performed under general anesthesia in an accredited surgical facility. Because this is a combination surgery, the recovery time can last a bit longer than a lift or augmentation alone. Most patients return to work within a few short days, but exercise and aerobic activities must be avoided for four to six weeks. It will take a few months for the breasts to settle fully into their new position and assume a soft, natural look. Women who have lived with sagging, deflated breasts are thrilled to be living with youthful, lifted breasts!

Will implants help sagging?

Breast augmentation enhances breast size but does not treat extensive sagging. If there is moderate to severe sagging, be leery of any surgeon who recommends augmentation with large implants to resolve the appearance of the breasts. Patients who are happy with their natural breast size but want to correct the sagging, can undergo a breast lift procedure without placing implants. Our surgeons may place internal surgical mesh to create the most beautiful, stable shape. The mesh is absorbable and is not permanent.

Which incision is best?

During your consultation, the incision placement will be discussed after your breasts have been thoroughly assessed. Women with mild sagging are typically candidates for a “lollipop” incision pattern. Larger breasts, or severely sagging breasts may be best treated with the anchor-shaped incision to create the most ideal outcome.

Long-lasting results?

Breast lifts are designed to last a very long time. If you maintain a healthy weight and do not get pregnant, the results are long-lasting. With time, aging skin and gravity may cause some natural sagging many years in the future and may inspire you to undergo a revision if you so choose.

What about scarring?

Unlike breast augmentation alone where the incisions are hidden in the breast crease, it is harder to conceal the incisions from a breast lift procedure, although they are carefully placed and will not be visible, even in a plunging bikini top or neckline. The scars fade very well, eventually appearing as a fine white line. Every effort is made during surgery to minimize scarring by not placing too large an implant and minimizing tension on closing incisions. With proper scar care and an avoidance of sun exposure, the scars will fade and become thinner over time. Most women feel that the beauty of the results far outweigh the scarring left behind.
