Women with maturing skin may have loss of skin elasticity and breast ptosis (sagging) and inquire about a breast lift procedure at our office. A common misconception with many women seeking plastic surgery is that a breast lift will make the breast bigger, but it will not. The lift will move the nipple areolar complex up and the breast gland can be redistributed to reshape the breast mound. In some cases, an internal mesh is necessary to support the gland and breast envelope. The implant is often necessary to reinflate the breast and give fullness to the upper pole in particular. If an implant is not done at the time of the lift, it can always be placed in the future if more volume is desired. Our board-certified plastic surgeons at Austin Plastic Surgery Institute can provide you with a private consultation in their office to see what type of augmentation and/or lift surgery would best serve your aesthetic goals. If you have lost volume in your breast from a significant weight loss or left with drooping breasts post-baby, our plastic surgeons can perform a breast augmentation with a lift in tandem. During the augmentation-mastopexy, the pocket is made for the implant and then the lift is done. The size of the implant must not be too large, or the excessive weight can pull against the repositioned breast and lead to asymmetry and malposition. A new opening is made in the skin to reposition the nipple areolar complex. It remains connected to the breast tissue to preserve its blood supply and sensation. For excessively large areolae, the diameter is made smaller so that it will be proportionate with the breast.
Breast Lift Incision
Breast lifts are performed with varying incision patterns. Some common incision patterns include:
- All the way around the areola (circumareolar/Benelli/donut mastopexy). This is less commonly used as it can result in a flattening of the breast and a widened areolar diameter
- Circumvertical/short scar/lollipop pattern results in a closure around the areola and an additional vertical line that will travel down the breast to the natural fold below the breast. These scars heal very well and preserve the rounder breast shape
- Full circumareolar, vertical, and horizontal incision (referred to as an anchor, inverted "T", or Wise pattern) is used where there is a lot of skin laxity or in cases of a very low nipple position
Breast augmentation with a lift can be performed as a single procedure (or operation). In rare cases it is broken into 2 separate procedures. The pocket that is created for the implant is the same as it would be for just straight forward augmentation; the only difference is the ultimate scar pattern. If it has been decided ahead of time that an implant will be placed during the lift procedure, it is usually positioned below the pectoralis muscle. This adds additional support to the breast and acts as an internal bra, which results in a longer-lasting outcome.
Breast augmentation with a lift is performed under general anesthesia in an accredited surgical facility. Because this is a combination surgery, the recovery time can last a bit longer than a lift or augmentation alone. Most patients are able to return to work within a few short days, but exercise and aerobic activities should be eliminated for at least 2-4 weeks. It will take a few months for the breasts to settle fully into their new position and assume a soft, natural look. Women who have lived with sagging, deflated breasts are thrilled with their new breast appearance.

Breast Aug With Lift FAQs
Will Implants Help Sagging?
Breast augmentation surgery will help with size, but will not lift a sagging breast. If there is moderate to severe sagging, women should be leery of any surgeon who recommends implants alone. Patients who are happy with the size of their breasts, but want to correct the sagging, will possibly benefit from a breast lift procedure (without the placement of implants). The use of the internal mesh is helpful in some patients to help create a better shape.
Which Incision is Best?
During your consultation, the incision placement will be discussed after your breasts have been thoroughly assessed. Women with mild ptosis will usually be candidates for the circumvertical/lollipop pattern. Larger breasts, or those that have severe sagging, may need the anchor-shaped incision in order to garner the best results.
Breast lifts are designed to last a very long time. As long as you do not experience a huge weight gain or become pregnant, the results can last for years. Age and gravity are unavoidable elements that will create some natural sagging throughout the years.
What About Scarring?
Unlike breast augmentation alone where the incisions can be carefully hidden, it is harder to conceal the incisions that are made during the lift procedure. Fortunately these scars fade very well over time. Every effort is made during surgery to minimize scarring by not placing too large an implant and minimizing any tension on the closure. With proper scar care and an avoidance of sun exposure, the scars will fade and become thinner over time. Most women feel that the physical results following this surgery far outweigh the scarring left behind.
